If you haven’t read Mike Monteiro’s Design is a Job, I would highly recommend it.
We need to fear the consequences of our work more than we fear the consequences of speaking up. We need to fear the consequences of our work more than we love the cleverness of our ideas. When the fear of taking that responsibility and the fear of getting in trouble is greater than the fear of causing harm, we stop becoming designers. We become agents of recklessness. When we create without thinking, we become death.
